As you may be aware, S4A have been working with CWP NHS Partnership Foundation Trust to secure some funding for adults with autism, for post diagnostic provision. CWP want to involve people with autism in deciding what they want services to look like.
I have attached a form below, for our ASC adults, to see if they would like to be involved in these discussions, which will likely take place over email. If you could complete and return the form, or if you could email CWP with your details, then they can add you to a database of people that are happy to be contacted in the near future.
