We have been asked by CWP NHS Foundation Trust for your feedback and comments if your child has been diagnosed with autism in the last 12 months at Space4Autism.

This is to help shape future service development.  If parents or children liked something – we would like them to tell us so that we can carry on doing it.  If parents or children did not like something about their experience – we would like them to tell us so that we can improve the service.  If you can write little or as much as you would like but please respond as soon as possible and before 31st March to info@space4autism.org and we will pass your comments onto the team

Please note that your comments will be passed on to the Special Educational Needs and Disabilities (SEND) officer for the local Clinical Commissioning Group, and they may be included in the next written statement of action monitoring meeting.
